Privacy Policy
YOUR FACE (hereinafter referred to as “the Clinic”) sets forth the following privacy policy (hereinafter referred to as “this Policy”) regarding the handling of personal information of customers (hereinafter referred to as “Users”).
Personal Information
“Personal information” refers to “personal information” as defined in the Act on the Protection of Personal Information and means information about a living individual that can identify the specific individual by name, date of birth, address, telephone number, contact information, and other descriptions included in such information, as well as data related to appearance, fingerprints, and voiceprints, and health insurance card insurer numbers and other information that can identify the specific individual from the information alone (personally identifiable information).
Method of Collecting Personal Information
The Clinic may ask Users for personal information such as name, date of birth, address, phone number, and email address when they make a reservation, etc. In addition, the Clinic may collect transaction records and payment information that include Users’ personal information between the Users and the Clinic’s partners (including information providers, advertisers, ad distribution destinations, etc., hereinafter referred to as “Partners”).
Purpose of Collecting and Using Personal Information
The purposes for which the Clinic collects and uses personal information are as follows:
- For the operation of the Clinic
- To respond to inquiries from Users (including identity verification)
- To provide necessary communications such as important notices
- For purposes incidental to the above usage purposes
Change of Usage Purpose
The Clinic may change the purpose of use of personal information only when the new purpose is reasonably deemed related to the original purpose.
If the purpose of use is changed, the Clinic will notify Users of the changed purpose by a method prescribed by the Clinic or announce it on the Clinic’s website.
Provision of Personal Information to Third Parties
The Clinic will not provide personal information to third parties without the prior consent of the User, except in the following cases or as permitted by the Act on the Protection of Personal Information and other applicable laws and regulations:
- When it is necessary for the protection of a person’s life, body, or property, and it is difficult to obtain the consent of the person
- When it is particularly necessary for improving public health or promoting the sound development of children, and it is difficult to obtain the consent of the person
- When it is necessary to cooperate with a national institution, a local government, or a person entrusted by such entities to perform duties prescribed by laws and regulations, and obtaining the person’s consent is likely to impede the performance of such duties
- When the following items are announced or made public in advance and the Clinic has submitted notification to the Personal Information Protection Commission:
- That the purpose of use includes provision to third parties
- Items of data provided to third parties
- Means or methods of providing the information to third parties
- That the provision of personal information to third parties will be stopped upon the User’s request
- The method of accepting such User requests
Notwithstanding the provisions of the preceding paragraph, the following cases do not fall under the category of third parties:
- When the Clinic entrusts all or part of the handling of personal information within the scope necessary to achieve the usage purpose
- When personal information is provided as a result of business succession due to merger or other reasons
- When personal information is jointly used with a specific party, and the User has been notified in advance or can easily access the following details: the items of personal information to be jointly used, the scope of parties jointly using the information, the purpose of use by such parties, and the name or title of the party responsible for managing the personal information
Disclosure of Personal Information
When a User requests disclosure of personal information, the Clinic will disclose it to the User without delay. However, if disclosure falls under any of the following categories, the Clinic may withhold all or part of the information, and will notify the User of the decision not to disclose without delay. Please note that a handling fee may be charged for disclosing personal information.
- When there is a risk of harm to the life, body, property, or other rights and interests of the User or a third party
- When there is a significant risk of hindering the proper execution of the Clinic’s operations
- When disclosure would violate other laws or regulations
Notwithstanding the provisions of the preceding paragraph, the Clinic will not, in principle, disclose information other than personal information, such as history or characteristic information.
Correction and Deletion of Personal Information
If a User believes that their personal information held by the Clinic is incorrect, the User may request the correction, addition, or deletion of such personal information (hereinafter referred to as “Correction, etc.”) according to the procedure established by the Clinic.
If the Clinic determines that it is necessary to respond to the request based on the User’s request in the preceding paragraph, the Clinic will carry out the Correction, etc. of the personal information without delay.
If the Clinic carries out the Correction, etc. based on the preceding paragraph, or decides not to make the Correction, etc., it will notify the User of the decision without delay.
Suspension of Use of Personal Information, etc.
If the Clinic is requested by a User to suspend or delete the use of personal information (hereinafter referred to as “Suspension of Use, etc.”) on the grounds that it is being handled beyond the scope of the intended use or was obtained by fraudulent means, the Clinic will conduct a necessary investigation without delay.
Based on the results of the investigation, if the Clinic determines that it is necessary to comply with the request, it will carry out the Suspension of Use, etc. without delay.
If the Clinic carries out the Suspension of Use, etc. based on the preceding paragraph, or decides not to do so, it will notify the User of the decision without delay.
Notwithstanding the preceding two paragraphs, if the Suspension of Use, etc. requires a large amount of cost or is otherwise difficult to implement, and alternative measures necessary to protect the User’s rights and interests can be taken, the Clinic will take such alternative measures.
Changes to the Privacy Policy
Unless otherwise provided in laws or in this Policy, the content of this Policy may be changed without notifying the Users.
Unless otherwise specified by the Clinic, the revised Privacy Policy shall take effect from the time it is posted on the Clinic’s website.
