以前の手術によって、鼻柱基部が頬に“刺さったように”見える変形を残していました。

この症例では、
局所皮弁＋筋膜移植を用いて、鼻腔底（土手）を再建しました。

Secondary Alar Base Reconstruction – Releasing the “Pinned Columella”
(3 months postoperative)

The patient presented with a deformity often seen after aggressive philtrum shortening:
the columellar base appeared to be “sunken” or “pinned into the cheeks”, with exaggerated nostril exposure.

We performed a nasal sill reconstruction using a local flap and fascial graft,
targeting the collapsed alar-columellar junction.

Postoperative outcome:

🔹 The columellar base regained its proper volume and projection
🔹 Nostril exposure reduced dramatically in frontal view
🔹 A natural transition between philtrum and nose was restored
🔹 Facial topography regained depth and balance

This subtle reconstruction supports what we call “the forgotten nose”—

術前の状態では、不自然に尖った鼻尖、過度の上向き（アップノーズ）、鼻尖直上の膨らみ（supratip fullness）、鼻背の軸の偏位が目立ち、

Complex Revision Rhinoplasty – Full Autologous Reconstruction Toward “The Forgotten Nose”

This patient presented with:
– An unnaturally projected, pointed tip
– Over-rotated “upturned” deformity
– Prominent supratip fullness
– Dorsal axis deviation creating asymmetry

The nose was visually dominant, disconnecting from the harmony of the face.

Our objective was not just correction, but reconstruction through reintegration.
We pursued a philosophy of the “Wasure-bana”—
